FINANCIAL REIMBURSEMENT STATEMENT

 

          All services provided are based on the patient’s condition and family circumstances as assessed and determined by the Interdisciplinary Team.  What appears below is the conjunction of Medicare and Hospice benefits; private insurance plans may vary.  Hospice will help you to determine the extent of your private insurance coverage and will provide you with a detailed explanation of your financial obligation to Hospice, if any.

Inpatient stays-short stay
for symptom/pain control

Continuous care at home during period of medical crisis

24 hour on-call capability

Covered Except for 5% Co-pay

Prescription medications related to the terminal diagnosis.  Hospice will bill patient / family for medication co-pay.  Maximum cost per single drug is $5.00.

Treatment of problems unrelated to terminal illness (standard Medicare will pay)

Primary physician's bills (patient's doctor will bill Medicare directly under Medicare Part B)

Unauthorized ambulance use (privately paid)

Unauthorized supplies & equipment (privately paid)
