Guidelines & Eligibility

          The purpose of hospice care is to enable individuals to be cared for at home, receiving relief from pain and other symptoms, in the one place where they can be most comfortable.  Anyone can refer a patient to hospice. Doctors, family members, co-workers, and friends are all eligible to make recommendations on behalf of someone else.  Hospice is an equal-opportunity organization.  No one will be denied consideration for services based on age, race, national origin, gender, religious affiliation, inability to pay, physical or mental handicap, or other unjust barriers.  When determining if you or someone you know may benefit from the services of Lewis County Hospice, please refer to these four admission guidelines:

1. The patient must reside in Lewis County.
2. The patient and doctor must agree to a
     non-aggressive course of treatment.
3. The patient must have a life expectancy in terms of
     months.
4. The patient must have a caregiver available 24
     hours a day
